Occupational Therapy Home Assessments are used to determine
the level of housekeeping, attendant care, and or care-giving
assistance that is required by an individual after they
have sustained an injury resulting from a motor vehicle
accident.

What’s included in the Home Assessment?
At TSR we perform thorough assessments and furnish
superior reports that will be of value to you. Here
is what you can expect from us:
- Direct, straight forward (not convoluted) answers
to referral questions
- A detailed review of medical history summarized
in chronological order in the report
- Clinical assessments including measurements of
range of motion and physical strength
- Results of clinical tests, home activity simulations
and consistency tests incorporated into easy-to-read
tables in the report.
What types of Home Assessments can be performed?

Assessment can be performed as an Independent Evaluation
conducted under section 42 of the Statutory Accident
Benefits Schedule (SABs)

Assessment may also be performed by a treating therapist
conducted under section 24 of the SABs.
Who conducts the Home Assessment?
A registered occupational therapist will conduct the
Home Assessment.
Where is the Home Assessment conducted?
The Home Assessment is conducted in the individual’s
place of residence. TSR Clinics can arrange to perform
a home assessment anywhere throughout Ontario.
How long does the Home Assessment take?
A Home Assessment typically takes 2 to 4 hours to complete.
When can I expect the report?
Reports are furnished between 5 to 7 business days,
with an attempt to meet urgent deadlines if requested.
